Modern Imaging of Urogenital Tuberculosis

Abstract

Albeit rare compared to respiratory forms of the disease, urogenital tuberculosis (UG-TB) is increasingly encountered even in developed countries, particularly in immunosuppressed individuals or immigrants from endemic regions. Genitourinary involvement may occur either isolated or in the context of lung or disseminated tuberculosis.

This chapter reviews the pathogenesis, clinical manifestations and imaging appearances of UG-TB affecting the kidney and collecting system, ureter and urinary bladder and male and female genital organs. Most of the emphasis is placed on multidetector CT, which provides a comprehensive assessment of the entire urinary tract including both morphological and functional information, even in nonfunctioning kidneys. CT is highly sensitive for both early and chronic tubercular changes. A combination of consistent CT findings strongly supports the challenging clinical and laboratory diagnosis of UG-TB, which requires prolonged pharmacological therapy and sometimes ablative or reconstructive surgical procedures.
