Transfer in Subscapularis Lesions: Open Surgical Technique

Latissimus Dorsi Transfer

Abstract

The most common described tendon transfer for irreparable subscapularis tendon tears is the pectoralis major tendon transfer. However, the reported outcomes of this transfer have not been uniformly successful. In particular, patients with anterior glenohumeral instability are more likely to have poor functional results. Because of the similarities of the posterior origin of the subscapularis and the latissimus dorsi muscles, and the similar line of pull of their muscle fibers, transfer of the latissimus dorsi tendon to reconstruct an irreparable subscapularis tendon is considered better anatomically and biomechanically. We present an open surgical technique for transfer of the latissimus dorsi tendon for irreparable subscapularis tendon lesions.
