Abstract

Advanced infiltrative hepatocellular carcinoma (HCC) is rarely treated by resection. According to the Barcelona clinic liver cancer (BCLC) algorithm, extensive multifocal disease with or without vascular invasion in patients with preserved liver function is defined as stage B with intermediate HCC or stage C with advanced HCC, and palliative treatments such as transarterial chemoembolization (TACE) and Sorafenib are considered the standard treatment in these stages. The recent Japanese multicenter study with propensity score matching confirms the findings regarding short-term clinical benefits of laparoscopic liver resection (LLR) over open liver resection (OLR) without any compromise of oncologic principles for HCC patients. We describe a case of advanced infiltrative HCC with microscopic portal vein thrombus treated by pure laparoscopic left hepatectomy, and discuss indications for resection of HCC, intraoperative technique, and alternative minimally invasive approach for HCC.
