Abstract

Twins account for approximately 3 % of all deliveries, but make a disproportionate contribution to overall rates of perinatal mortality and morbidity. Twin pregnancies are at increased risk of preterm delivery, hypertensive disorders, gestational diabetes, congenital anomalies, stillbirth, and growth restriction compared with singleton pregnancies. Monochorionic twins are at significantly higher risk of adverse perinatal outcome than dichorionic twins. They are also at risk for unique pregnancy complications due to vascular anastomoses between the circulations supplying each of the twins. This chapter reviews most common late pregnancy complications associated with twins, as well as current recommendations on their prevention and management. It also provides an evidence-based guidance on optimal timing and route of delivery of twins.
