Practical Guide to Paraphilia and Paraphilic Disorders

Abstract

The 5th Edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM 5) classified pedophilia as a paraphilic disorder and the World Health Organization in 10th Edition of the International Classification of Diseases as a sexual preference disorder. Pedophilia is a chronical condition that usually begins in youth or the beginning of the adulthood and persists throughout life. This disorder is characterized by the presence of behaviors, fantasies and/or recurring sexual thoughts, intense and exciting involving children of up to 12 years old, over a period, equal or superior to 6 months. The etiology of this disorder remains poorly clarified, although some studies suggest the involvement of neurological, hormonal, and psychodynamic modifications. The treatment of the pedophilic disorder includes psychotherapy, pharmacological and surgical approaches. The evidence favors the treatments with antiandrogenic medications and the psychotropic drugs. Future controlled studies are warranted to produce improved evidence regarding the efficacy and duration of the pharmacologic treatment.
