Diagnostic Testing in Allergic Diseases

Abstract

When a medical history is suggestive of an allergic cause, proper testing is performed to identify clinically relevant allergens. The discovery of IgE antibody, along with the recognition of its central role in the allergic reaction, has established the identification of allergen-specific IgE as the key to the diagnosis of allergic diseases. Both in vivo skin testing and laboratory allergy testing are designed to detect allergen-specific IgE, although the pathophysiology of allergy is far more complex than just an IgE-mediated condition. Diagnostic testing for nonIgE-mediated allergic reactions and for allergies with predominant late-phase reaction and challenge testing with food or other allergens are beyond the scope of this chapter. Likewise, diagnostic tests for autoimmune conditions and immunodeficiencies are discussed in other chapters.
