Abstract
Reconstruction of the nipple-areolar complex (NAC) is the final step in breast reconstruction and represents the search for symmetry between both breasts. Areola reconstruction with local skin grafting technique improves the texture and appearance of the reconstructed areola, therefore giving it a more natural look. Once the position of the new nipple is determined, markings are made for the contralateral areola. Then the external limits of the new areola are incised and the skin is centripetally deepithelialized up to 85 % of its diameter. After this procedure, the detached skin is repositioned in its bed as a graft. Incisions are made in V and C forms, so that at the end of the healing process they will result in changes in the areolar texture. After 3 months, tattooing is performed so that its pattern simulates the wrinkles of the normal areola. At this step, different ink shades are used to better reproduce the various tones of the normal areola and give it a three-dimensional appearance. The good aesthetic results presented by these patients were determined by these procedures, without the inconvenience of grafting at a distance.
