Abstract

Nitrate therapy is the oldest treatment modality for angina pectoris. In addition to sublingual nitroglycerin, which is commonly used to relieve acute episodes of angina pectoris, longer-acting preparations for oral use are also available and used to prevent the occurrence of angina. The antianginal effect of nitrates is related to both their dilator actions on veins and arteries, which reduce myocardial oxygen demand. In addition, these agents can exert a vasodilator effect on the coronary tree thus helping to maintain or increase coronary artery blood flow. In this chapter we review the role of nitrates as anti-anginal agents and discuss their mechanism of action and the problem of tolerance and side effects.
