Abstract
Patients receiving chronic anticoagulation often require interruption of treatment perioperatively. It remains challenging to mitigate both the risk of thromboembolic events off anticoagulation and the risk of surgical bleeding on anticoagulation. The underlying condition requiring anticoagulation impacts the decision of whether to interrupt long-acting agents such as warfarin or to use bridging therapy with shorter-acting agents.
