Endoscope-Assisted Microsurgery of the Corpus Callosum

Abstract

Surgical approaches to the corpus callosum (CC) are very challenging because of its deep and central position. This applies not only to approaches where the CC is the target, for example, corpus callosotomies, but also where it is only a gateway to deeper structures, as in transcallosal approaches. Endoscope-assisted microsurgical techniques offer many valuable features for CC surgery and are receiving increasing attention and use. Such features – better visualization of the surgical field and adjacent structures with improved magnification – have resulted in decreased brain retraction and better outcomes for the patient. Nevertheless, some disadvantages for the surgeon have to be considered. In this chapter, we will analyze the application of the endoscope in microsurgical approaches to the CC. We will describe two types of surgical technique, one involving the CC as the target (as in endoscopic-assisted corpus callosotomy) and the other with the CC as a gateway to other structures. We will discuss the advantages and disadvantages of this surgical modality and consider future uses.
