Zusammenfassung

Komplikationen bei der Resektion von Kiefer- und Gesichtstumoren umfassen ein breites Spektrum allgemeiner, medizinischer wie auch lokaler Komplikationen der chirurgischen Manöver. Sie können vielfältige Ursachen im Rahmen der Vorbereitung, bei der chirurgischen Resektion selber oder postoperativ haben. Die Häufigkeit medizinischer Komplikationen ist nicht nur aufgrund der Dauer und Komplexität der Eingriffe, sondern auch aufgrund der häufigen gesundheitlichen Probleme von Tumorpatienten hoch. Sie erfordern daher eine sorgfältige Auswahl der Therapieverfahren und entsprechende Vorsichtsmaßnahmen. Unter den spezifischen lokalen Komplikationen dominieren Wundheilungsstörungen, die neben einer funktionellen Verschlechterung auch zur Verzögerung adjuvanter Therapiemaßnahmen und letztlich zu einer Verschlechterung der Prognose des Patienten führen können. Hier sind insbesondere die Komplikationen von Kontinuitätsresektionen des Unterkiefers und die verschiedenen Formen der kontinuitätserhaltenden Resektionen zu nennen. Adjuvante Therapien spielen eine besondere Rolle und müssen je nach Tumorstadium berücksichtigt werden, da sie auch Einfluss auf die Ausprägung chirurgischer Komplikationen haben.
