Zusammenfassung

Dysplasie und Instabilität des Hüftgelenks gehören zu den anspruchsvollsten kinderorthopädischen Erkrankungen, die die Biomechanik verändern und in der Folge zu Muskelüberlastung, Arthrose, Schmerzen und eingeschränkter Mobilität führen. Die Ursachen sind entwicklungsbedingt, angeboren und in der Mehrzahl neuromuskuläre und systemische Erkrankungen. Eine Verbesserung der Deckung des Hüftkopfes und der Kongruenz des Gelenks kann durch Osteotomien an der Hüfte im Kindesalter erreicht werden, solange die Plastizität des muskuloskelettalen Gewebes gegeben ist. Die Auswahl des am besten geeigneten chirurgischen Verfahrens hängt vom Pathomechanismus, der Form der Hüftpfanne sowie vom Alter und den Aktivitäten des Patienten ab. In diesem Kapitel werden Becken- und Oberschenkelosteotomien beschrieben, die sich unter Berücksichtigung von Langzeitstudien als wirksam und sicher erwiesen haben.
