Zusammenfassung
Das Hüft-Resurfacing ist ein wirksames Verfahren, das jüngeren Menschen ein höheres Maß an Aktivität ermöglicht. Probleme mit einem hohen Gehalt an Kobalt- und Chromionen bei einigen Fabrikaten haben zu ernsthaften Bedenken gegenüber Metall/Metall-Gleitpaarungen im Allgemeinen geführt. Keramik/Keramik-Gleitpaarungen werden seit Jahrzehnten in der Hüftendoprothetik eingesetzt, und der neueste „Keramikverbundstoff Biolox Delta“ wird seit über 10 Jahren verwendet. Die endoprothetische Fixierung mit plasmagespritztem Titan und/oder Hydroxylapatit ist ebenfalls seit mehr als 10 Jahren weit verbreitet. Wir haben diese drei Technologien zu einem anatomischen zementfreien keramischen Oberflächenersatz, der H1, kombiniert. Die Konturen dieses Implantats wurden von experimentellen Arbeiten an den Rändern der nativen Hüfte abgeleitet und so modifiziert, dass die Herstellung mit dem Delta-Keramikverbundstoff möglich ist. Es wurden umfangreiche präklinische Labortests durchgeführt, Instrumente für den Einmalgebrauch entwickelt und anschließend klinische Studien durchgeführt. Fünfundneunzig Patienten wurden mit dem keramischen Oberflächenersatz operiert und bis zu 2 Jahre lang beobachtet. Bislang sind die Ergebnisse vielversprechend, und die klinische Studie wird fortgesetzt.
