Annual Update in Intensive Care and Emergency Medicine 2023

Abstract

In this chapter we will review the general aspects of acute respiratory distress syndrome (ARDS) in pregnancy including obstetric and non-obstetric risk factors. The main characteristics of ARDS induced by COVID-19 in pregnancy will then be discussed as well as details on lung mechanics and potential strategies to ventilate these patients including prone positioning. Pharmacologic treatment in pregnant patients with COVID-19 (e.g., steroids, immunotherapies, antivirals, and thromboprophylaxis) will also be discussed, as well as the indications for the timing and mode of delivery and the role of extracorporeal membrane oxygenation (ECMO) to manage refractory cases.
