Abstract

Intracranial arachnoid cysts (ACs) are an uncommon multifaceted pathologic entity that is more complex than it was previously. These cystic lesions may appear with variable clinical signs and symptoms as well as neuroimaging appearances which depend principally on their anatomic locations, cyst size, etiological forms, and the patient’s age. Some cysts are asymptomatic, while others manifest as a space-occupying lesion. ACs may be confused with many normal anatomic variants and intracranial cysts of different etiologies. Both neurosurgeons and radiologists must be prepared to recognize all possible considerations on preoperative images. Therefore, a high index of suspicion needs to be kept in mind to achieve an accurate analysis of ACs and to generate the list of the potential intracranial cyst-mimicking lesions. A summary of the most common imaging differential diagnoses for intracranial ACs is given in this chapter according to the anatomic localizations and origin of the cysts.
