Principles of Perioperative Management in Acute Care Surgery

Abstract

Perioperative management of the acute care surgical patient can present a complicated array of management decisions. Incorporating the principles of Advanced Trauma Life Support provides the surgeon with a consistent systematic approach for treatment. Identification of the different shock states and their management is key to caring for this often critically ill population. Utilization of current guidelines and protocols such as the Surviving Sepsis Campaign, Acute Respiratory Distress Syndrome Network, and massive transfusion protocols is crucial to optimize outcomes as well. Endpoints of resuscitation can help to guide the surgeon’s management and provide goals in the pre- and postoperative setting. These endpoints may require invasive procedures, and all have limitations that should be recognized.
