Ruptured Abdominal Aortic Aneurysm (rAAA)

Abstract

Ruptured abdominal aortic aneurysm (rAAA) is immediately life-threating and carries an extremely high mortality risk. It mostly involves elderly patients, and, even if surgery is performed within a very short time, mortality and morbidity are very high. The introduction of endovascular aortic repair (EVAR) indicates a paradigm shift in the treatment of rAAA, and it is now recommended, where possible, in major vascular guidelines. EVAR for rAAA treatment is the first-line recommendation within Endovascular Resuscitation and Trauma Management (EVTM). Upon diagnosis, acute open or endovascular surgery by a highly experienced team should be conducted, with the preferred method depending on center experience, anatomical considerations, comorbidities, and age of the patient. Peri- and postoperative intensive care treatment is of the utmost importance for successful recovery. This chapter will provide an up-to-date summary of the diagnoses and treatment possibilities for ruptured abdominal aortic aneurysm. It aims to give current general information and background for diagnosis and treatment of rAAA as used worldwide.
