The Interface Between Therapeutic Gastrointestinal Endoscopy and Endoscopic Gastrointestinal Surgery

Abstract

The era of modern flexible gastrointestinal endoscopy began in the mid-twentieth century. The modality has evolved remarkably since that time both in terms of technique and technology. Initially a diagnostic tool, endoscopy rapidly gained a therapeutic dimension. Gastroenterologists rapidly embraced this specialty and made vast contributions to the appreciation of pathology and refinement of technique. Gastrointestinal surgeons have also been committed to endoscopy since its introduction and have been in large part responsible for conceptualizing and introducing new therapeutic applications of endoscopy, including polypectomy, endoscopic gastrostomy, variceal banding, and ERCP. Although there has been competition and occasional acrimony between gastroenterologists and surgeons over who should most appropriately perform these procedures, perhaps the greatest advances have and will occur when the two specialties collaborate to advance the field.
