Abstract
Ambiguous genitalia discovered in the delivery room can be a stressful situation for the parents and care providers. The differential diagnosis of disorders of sexual development (DSD) includes genetic etiologies, disorders of steroid hormone production, as well as exogenous hormone exposure. Life-threatening conditions such as congenital adrenal hyperplasia must be identified. In the case of partial androgen insensitivity, a multidisciplinary team is instrumental in helping the parents and the individual manage the decisions including potential genitoplasty, gonadectomy, and hormone replacement at puberty. As these issues are influenced by the individual’s ultimate gender identity, deferring these decisions until adolescence may be advantageous.
