Functional Ear, Nose, and Throat Disturbances in Children and Adolescents

Abstract

This chapter focuses on the manifestations of distress through a number of unexplained symptoms in the acoustic system, voice system, and throat, seen in the clinic of the ear, nose, and throat specialist. A number of common presentations are described, like functional hearing loss or pseudohypoacusis or the phenomenon of phonophobia or hyperacusis. Differential diagnosis and management strategies are covered. The problems associated with the throat and the voice include globus hystericus, unexplained dysphonia, and foreign accent syndrome. Vertigo of a psychogenic nature and benign paroxysmal syncope are described, as well as psychogenic dizziness. A common thread in all these conditions is that the clinical presentation cannot be explained by any structural or anatomical damage, frequently a sudden onset and improvement when some psychosocial factors are alleviated. We describe the presentations with several clinical vignettes, illustrating stress in the life of the child, perfectionism, difficulty speaking of feelings of tensions, and, in particular, hostility and fear. A multimodal therapeutic approach is described.
