Abstract

Anterior instability is the most common kind of shoulder instability, resulting from traumatic abduction and external rotation. Initial treatment is nonoperative in the general population. This consists of closed reduction, immobilization, and physical therapy. Special considerations in contact/collision athletes and military personnel include early return to activity and early operative intervention. Arthroscopic Bankart repair is most commonly selected for initial stabilization in the United States. Risk factors for recurrent instability include generalized laxity, young age, bone loss, collision sports, and frequent instability events. Open Bankart repair may be the more successful initial management in these cases. Latarjet coracoid transfer is performed following failed stabilization surgery or for large glenoid bone loss.
