Abstract
An 87-year-old man presented with a month history of enlarging, tender skin ulcerations of both lower extremities. He had a history of multiple myeloma that was treated by Lenalidomide. Three weeks after chemotherapy, the patient noticed painful inflammatory pustules on the legs, some of them were ulcerated and painful after minor trauma. Other medical history included congestive heart failure, hypertension, common stenosing atherosclerosis, chronic renal dysfunction and benign prostatic hyperplasia (BPH). Dermatologic examination revealed multiple ulcers of the lower limbs with a peripheral red halo with raised, red-purple, undermined edges. Histopathological examination revealed the dense neutrophilic infiltrate in the dermis with signs of leukocytoclastic vasculitis and formation of subepithelial microabscesses, necrosis of individual collagen fibers. Measurement of plasma levels of adenosine deaminase 2 showed negative results.
