Ovarian Cancer Screening and Early Detection

Abstract

Ovarian cancer is the most deadly gynecologic malignancy due to the fact that most patients have advanced disease at the time of diagnosis. Early ovarian cancer is not associated with identifiable symptoms and screening efforts have not been able to significantly impact the mortality from this malignancy for multiple reasons. While work has been directed toward developing more sensitive tests, ovarian cancer appears to develop in the fallopian tube and will spread diffusely in the abdominal cavity prior to detection. Single agent biomarker tests and even multimodal tests that incorporate multiple tumor markers and ultrasound have been developed. While these types of tests are relatively sensitive for detecting the presence of ovarian cancer in women when a mass develops, they do not perform well as a screening test in asymptomatic women. While some tests can detect some ovarian cancer at an earlier stage, they are not cost-effective and are not likely to decrease the overall mortality significantly. It is hoped that ovarian cancer precursors can be detected using cytology or by examining molecular signatures to provide early detection and reduction of mortality from ovarian cancer.
