Robot-Assisted Radical Prostatectomy: The Extraperitoneal Approach and the Future with Single Port

Abstract

The robot-assisted radical prostatectomy (RARP) is a safe and effective surgical procedure for prostate cancer that is associated with decreased blood loss, and shorter hospital stays when compared to the open approach. The extraperitoneal (EP) RARP avoids entry into the peritoneal cavity, and allows for minimally invasive surgical treatment of prostate cancer, which is advantageous for patients who desire surgical therapy with multiple prior intraabdominal surgeries. The difficulties in performing multiport EP RARP include creating the EP space, and a smaller working space for the robotic arms. The Single-Port robot minimizes these difficulties as creation of a wide extraperitoneal space required for the multiport robotic arms is not necessary. In this chapter, we review the steps and our experience with EP RARP, using both the multiport and Single-Port da Vinci robotic systems, with images to highlight critical steps.
