Abstract
The first phakic IOLs were implanted into the anterior chamber by Strampelli as early as in 1953. Since then, numerous incremental improvements and various models have been introduced, improving tolerability and surgical safety. This chapter gives an overview of the currently available types of phakic IOLs and explains their implantation in a step-by-step fashion. Today, both modern phakic posterior chamber lenses and iris-fixed anterior chamber lenses provide safe and effective correction of particularly high ametropia. This is particularly helpful when refractive laser approaches for the cornea are contraindicated, e.g., in case of keratoconus or high hyperopia and myopia. The implantation can be technically demanding however, and it should be remembered that, given enough time, these lenses are likely to require explantation.
