Controversies in Orthopedic Surgery of The Upper Limb

Abstract

When proximal interphalangeal (PIP) and metacarpophalangeal (MCP) joint arthroplasties are performed, it is paramount to have bone and soft tissue stability for the implant to work properly. Therefore, the treatment of each patient will depend on the soft tissue envelope and the amount of joint destruction. Primary MCP joint arthroplasty may be unconstrained (pyrocarbon), of silicone, a surface replacement arthroplasty (SRA), or a dorsal capsule interpositional arthroplasty. When a primary arthroplasty of the MCP joint fails, revision MCP joint arthroplasty will be required. Primary PIP joint arthroplasty with silicone implants may be performed as urgent surgery in complex fractures. PIP joint arthroplasty can be carried out with surface-replacing implants, silicone implants, or pyrolytic carbon implants.
