Transcranial Doppler in Acute Bacterial Meningitis

Abstract

Cerebrovascular complications of acute bacterial meningitis have been well recognized since more than five decades ago when initial studies gave a glimpse of the regional and global alterations of cerebral blood flow and its relation to neurological deficit and clinical outcomes. Despite the advances on understanding the pathophysiology and wide availability of antimicrobial treatment, the rate of complications and poor outcomes is persistently high. One of the likely reasons for this is the occurrence of cerebrovascular events limited not only to stroke but also venous sinus thrombosis, vasospasm, stenosis, loss of autoregulation and vasoreactivity and hydrocephalus. Transcranial Doppler (TCD) is a bedside non-invasive monitoring tool with a potential role in the approach and individualized treatment of central nervous system (CNS) infections, especially in terms of hemodynamic management and prognostication. This chapter will explore the available evidence and practice of the use of TCD in the setting of acute meningoencephalitis.
