Acute Hematogenous Osteomyelitis

Abstract

Children with acute hematogenous osteomyelitis demonstrate a wide range of illness, ranging from mild to severe. Long-term clinical outcomes correlate with severity of illness at the time of initial hospital presentation. The treatment of acute hematogenous osteomyelitis utilizes a strategy of sequential parenteral to oral antibiotic therapy. Some children require surgical intervention for source control. Given the evolution of antibiotic resistance among Staphylococcus aureus isolates in different communities, it is increasingly relevant to obtain a sample from bone by trocar or during open irrigation and drainage procedures to guide specific antibiotic selection. This chapter reviews recent evidence pertaining to the evaluation and treatment of children with acute hematogenous osteomyelitis.
