Abstract

Zonulopathy significantly increases the risk of surgical complications with phacoemulsification. Strategies and modifications for each surgical step are discussed in this chapter, including the indications and timing for capsule retractors and capsular tension rings. The risk of postoperative IOL decentration and bag-IOL dislocation is also increased in these eyes. The underutilized strategy of ciliary sulcus fixation of a three-piece foldable IOL is outlined.
