The Cecil-Culp Technique

Abstract

Cecil in 1946 reconstructed new urethra from the urethral plate and the surrounding penile skin. He then buried the neourethra into a scrotal incision that he made to allow for more vascularity and tension free healing and performed perineal urethrostomy at the same time.

Culp modified Cecil technique by performing chordee repair at the first stage without urethroplasty. In a second stage, he created a neourethra using a Thiersch-Duplay reconstruction and buried the penis into the scrotum and left a urethral catheter instead of creating a perineal urethrostomy. He separated the penis from the scrotum in a third stage.

Nowadays, the Cecil-Culp (CC) technique has resurrected as an ideal salvage procedure in conjunction with buccal mucosal grafts for redo complex penile surgery when the vascularity is compromised due to multiple failed surgeries and severe scarring and fibrosis.
