Sentinel Node Navigation Surgery for Upper Gastrointestinal Cancer

Abstract

Radical resection with systematic lymph node dissection is traditionally performed in patients with upper gastrointestinal cancer because the lymphatic system of these cancers is complex and skip metastases are often present. Although sentinel lymph node mapping for upper gastrointestinal cancer is still in clinical trials, the sentinel node concept could be applied for early-stage cancers. In early gastric cancer, in particular, cancer metastasis can be identified with high diagnostic accuracy using the dual tracer method and sentinel basin dissection. The usefulness of sentinel node navigation surgery for early gastric cancer has been verified in multicenter prospective studies. In the near future, it will likely be possible to safely perform radical surgery with a minimal extent of gastrectomy based on the sentinel node concept.
