Peterson’s Principles of Oral and Maxillofacial Surgery

Abstract

Moderate-to-severe occlusal discrepancies and dentofacial deformities in adolescents and adults usually require combined orthodontic treatment and orthognathic surgery to obtain optimal results. This chapter presents the methods for diagnosing dental, dentoalveolar, and skeletal factors that may need to be addressed orthodontically in planning for orthognathic surgery. These factors may require specific orthodontic approaches in preparation for surgery. The orthodontic goals are to position the teeth over basal bone; avoid excessive intrusion, extrusion, expansion, tipping, and torquing; avoid unstable orthodontic mechanics; and provide stable and predictable orthodontic preparation. Surgical alteration of the occlusal plane may be indicated for optimal functional-aesthetic outcomes and can affect the presurgical orthodontic setup. Postsurgical orthodontics are equally important to finalize the dentofacial and occlusal relationship. Aggressive orthodontic mechanics may be required to maximize the occlusal fit. The protocols included herein should help the clinician provide appropriate pre- and postsurgical orthodontics. Coordination of the orthodontics with the specific orthognathic surgery is important for high-quality outcomes.
