Abstract

Targeted intrathecal drug delivery (TIDD) is an established analgesic modality in the management of intractable chronic cancer and noncancer pain. While prospective studies on TIDD are scant, this form of therapy is often used when other treatments have failed. Clinicians administering TIDD need to appreciate a number of factors that are important for the successful application of the therapy. These include understanding of cerebrospinal fluid flow dynamics, pharmacokinetics and pharmacodynamics of intrathecal agents, pain and patient characteristics in order to deliver the most optimal treatment to a particular patient. This chapter addresses, in a succinct manner, the salient features of TIDD including basic and clinical parameters that determine outcome. Additionally, potential novel agents and applications are discussed.
