Excessive Daytime Sleepiness: An Overview

Abstract

Excessive daytime sleepiness is defined as the need to sleep during the day or at inappropriate times. Hypersomnia is closely related to loss of ability to concentrate, irritability, and loss of functionality. The differential diagnosis of hypersomnia is a medical challenge and involves a detailed interview and reasoning for the main groups of clinical entities. The main causes of hypersomnia are sleep deprivation, sleep disorders, clinical and psychiatric illnesses, medications, circadian rhythm disorders, and central hypersomnia. Symptoms can be quantified using clinical scales or objective neurophysiological tests. Well-validated scales help in the diagnosis and stratification of severity. Objective confirmation is performed through the Multiple Sleep Latency Test. Not all cases require additional tests. The treatment of hypersomnia is directed at the underlying cause. In some cases, general orientations and improving sleep hygiene habits may be sufficient. In other cases, the use of stimulant medications may be necessary.
