Abstract

The transradial arterial approach (TRA) has an established history of increased patient satisfaction and decreased complication rates for coronary procedures compared with the transfemoral arterial approach (TFA). Led by its well-documented safety profile in cardiac interventions, the TRA has become more widely adopted among the IR community for noncoronary interventions over the last decade. However, it is important for operators to remember that the TRA is not entirely benign and possesses its own set of complications and obstacles. Complications associated with the TRA include radial artery spasm, radial artery occlusion, access-site hematomas, and various vascular, neurologic, and device-related injuries. While the majority of TRA complications are not clinically dramatic, some require dedicated management, and many can be prevented with the appropriate patient selection and technique.

In this chapter, we detail potential complications and roadblocks operators may face when using the TRA during IR procedures, preventive measures, and the appropriate procedural and post-procedural management of these complications.
