Abstract

Aneurysmal subarachnoid hemorrhage is considered as one of the most devastating neurosurgical emergencies with mortality rate of 60% and more than one third of those who survive have major neurological deficit. Neurological complications of aSAH such as vasospasm and re-bleeding are most important causes of poor outcome. It is a heterogeneous disorder with a varied range of original presentations and final outcomes. Development of reliable grading scale of aSAH is extremely important which helps treating physicians to predict the outcome and complications, so treating physicians can take appropriate decision in appropriate time as well as to use the resources wisely and effectively. Moreover, grading scales facilitate communication between health care provider and patient’s evaluation throughout medical as well as research centers. This chapter discusses the requirements for ideal grading system, overview of role, and limitations of the most commonly used clinical and radiological grading system of aSAH (Rosen and Macdonald, Neurocrit Care, 2: 110–8, 2005, Steiner et al. Cerebrovasc Dis. 35: 93–112, 2013).
