Partial Trapeziectomy and Soft Tissue Interposition

Abstract

Through smaller, improved equipment and refined surgical techniques, hand surgeons are now able to address the entire spectrum of basilar joint arthritis arthroscopically. Specifically, arthroscopic resection arthroplasty for isolated carpometacarpal (CMC) or pantrapezial arthritis with interposition has been shown to provide excellent results and rapid recovery. Pantrapezial arthritis (Stage 4) disease no longer mandates open trapeziectomy. This chapter provides a detailed description of patient selection and surgical indications as well as step-by-step instructions of arthroscopic resection arthroplasty techniques and necessary equipment used by the author for isolated carpometacarpal or pantrapezial arthritis. A treatment algorithm is presented based on four important pre-operative variables: (1) CMC cartilage status, (2) CMC stability, (3) scaphotrapeziotrapezoid (STT) cartilage status, and (4) metacarpophalangeal pathology. A modification of an arthroscopic staging system is proposed. A new technique for arthroscopic stabilization of the CMC is presented.
