Abstract

Renal replacement therapy in the setting of AKI is frequently life-saving therapy. However, while there are some clear absolute indications for the initiation of renal replacement for patients with end-stage kidney failure, the optimum timing, dose and modality choice for patients with AKI remain controversial.

What is clear is that one size does not fit all and that renal replacement therapy needs to be tailored to the patient and the illness(s) in the context of their AKI. This is particularly important in the context of the unstable patient, those with cardiac or acute brain injury and electrolyte abnormalities, the details of which are covered in this chapter.
