Challenges in Pediatric Kidney Transplantation

Abstract

Kidney disease in children with rheumatic conditions can cause significant morbidity and mortality. Systemic lupus erythematosus (SLE), IgA vasculitis (IgAV), and ANCA-associated vasculitis (AAV) are the most common rheumatologic conditions that cause end-stage renal disease (ESRD) in children. There are many challenges to managing severe kidney disease in pediatric patients. Adequate research studies are lacking to provide good evidence-based guidelines for disease therapy and management of ESRD specific for this population due to the low incidence and prevalence of pediatric patients with ESRD from rheumatologic diseases. Here, we summarize clinical outcomes of renal involvement, ESRD, and renal transplantation and the many challenges that exist in managing pediatric patients with SLE, IgAV, and AAV. In general, good outcomes of renal transplantation are reported; thus rheumatologic disease alone should not exclude pediatric patients from renal transplantation.
