Abstract
A 52-year old man presented with a few month history of recurrent left epistaxis and left nasal obstruction. An nasal endoscopy revealed a pigmented soft tissue mass in the nasal cavity. This was confirmed by CT (Figs. 40.1 and 40.2) and MRI imaging. A biopsy confirmed the lesion to be a malignant melanoma. This was excised through a total septectomy via a lateral rhinotomy approach (Fig. 40.3). A year later, he developed a satellite malignant melanoma lesions in the posterior ethmoid and sphenoid sinuses (Fig. 40.4). These were treated by endoscopic excision followed by postoperative radiotherapy. The patient remained free of disease but passed away of unrelated causes 9 years after his initial presentation.
