Abstract

Acute aortic intramural hematoma is a subtype of an acute aortic syndrome in which blood is present within the aortic wall without evidence of intimal injury. The etiology of aortic intramural hematoma is unknown and the pathology may in fact be similar to a true aortic dissection in the absence of a visible intimal defect on imaging. Classification follows that of the Stanford classification system for aortic dissection. Management strategies remain controversial. Western centers generally follow similar management strategies as would be done for a conventional aortic dissection, whereas Asian centers have reported excellent outcomes with medical management.
