Treatment of Medial Platysma Through Lateral Approach

Abstract

Introduction: The aged neck presents important anatomical alterations that deserve special treatment in rhytidoplasty surgery. A technique using bilateral approach and with a medial section of the half platysma muscle are the reason for this chapter.

Methods: A lateral approach with submuscular dissection of platysma is performed, from its lateral edge to its half, from here, with scissors the medial half is cut through its thickness, going until the complete release of its medial border and detachment, by blunt dissection, from the suprajascente skin to the cut muscular edges, allowing an opening between them and undoing the medial muscular flanges. The lateral border of the muscle is tractioned and sutured to the muscular fascia of the sternocleidomastoid muscle. Complications: Temporary paresis of the mandibular branch of facial nerve.

Discussion: The authors discuss the possibility of having a simple surgery done for aesthetic result of the anterior neck.

Conclusion: With the technique presented in this chapter, the authors obtained in the last decade better results and fewer recurrences than when they used more radical techniques, in those previous three decades.
