Abstract
Introduction: Of the structural changes that age and weight loss promote on the face, volumetric changes in fat in the superficial and deep structures are the main factors. Absorbable synthetic fillers produce good results but must be applied at least once a year for maintenance. Fat is the only perfect biocompatibility substance, its use allows great results that will age with the patient, rarely needing new grafts.
Objective: To demonstrate that the fat graft recomposes the lost facial volume and the expansive anesthesia technique of the tissues allows safe access to the superficial and deep adipose structures, by facilitating the passage of needles and thin cannulas for aspiration and fat grafting more safely, with low trauma, and producing great results.
Method: Demarcation of the donor area and recipient areas of the face for infiltration with expansion of the subcutaneous and fatty compartments, temporal, and malar; the greater the distention of the recipient area, the greater the number of new cells that can be grafted with little impact and trauma, produced by movement of cannulas and needles in a distended water environment; this leads to a large three-dimensional increase in the tissues already grafted and when the anesthetic is absorbed, and the new cells will be deposited in a stable manner in the areas and levels determined with low inflammatory process in the postoperative period.
Conclusion: The anesthetic infiltration, very diluted and at low temperature, produces distension of the tissues, facilitating liposuction and allowing adipose tissue grafting. The transfer of new adipose cells to aged areas stimulates the formation of neovascularization, among other repair factors, achieving great improvement in the volume and quality of the skin.
Result: Due to the three-dimensional increase, liposuction with thin cannulas in expanded areas presents a high degree of retraction; the replacement of facial volumetry at the subcutaneous level and in the temporal and malar fat compartments, submitted to fat grafting, bring the balance of the face complementing the rejuvenation.
