Renal Artery Stenosis: State of the Art in the Diagnosis and Management

Abstract

Renal artery stenosis as defined by narrowing of one or more of the renal arteries by more than 50%, can cause significant morbidity and mortality in those patients predisposed to atherosclerotic disease. Clinical manifestations include recurrent pulmonary edema, unstable angina, renovascular hypertension, and ischemic nephropathy, which are associated with worse patient outcomes. While non-invasive methods such as duplex ultrasound and computed tomography can diagnose renal artery stenosis, invasive angiography remains the gold standard. Medical therapy is sufficient in a majority of patients; however a small subset of patients will benefit from an invasive approach. We will review the data underlying decision making strategies in this high risk patient cohort.
