Principles and Practice of Maternal Critical Care

Abstract

Renal failure during pregnancy and the peripartum period encompasses acute kidney injury (AKI), chronic kidney disease, and end-stage renal disease (ESRD). The epidemiology of these conditions varies worldwide. Acute kidney injury may be due to conditions specific to pregnancy, disorders that coincide with pregnancy, or an exacerbation of a pre-existing renal disorder. Management consists of prompt resuscitation of hemodynamic and fluid status, treatment of the underlying disease, and renal replacement therapy (RRT) in case of complications of AKI.

This chapter summarizes the most common causes of AKI; describes therapeutic options, including RRT; and emphasizes the need for close multidisciplinary management during the acute period and beyond.
