Approach to the Failed Hip Surgery

Abstract

The recent recognition of femoroacetabular impingement (FAI) has coincided with technological advances to contribute to the increase in the annual volume of hip arthroscopy. Although hip arthroscopy has the potential to provide clinical improvements and a reduction in pain, up to 10% of patients require subsequent procedures due to continued symptoms or persistent pain. The workup of these patients following failed hip arthroscopy includes a thorough history, physical examination, and imaging as appropriate. Although there are many etiologies of recalcitrant symptoms after hip surgery, the causes may be broadly categorized as follows: surgical indication, patient-specific factors, rehabilitation-related factors, or technical considerations. After determining the specific cause of continued symptoms, these patients may be managed with nonoperative treatments, revision hip arthroscopy, hip resurfacing, or total hip arthroplasty (THA). This chapter provides an outline of the diagnosis, causes, and management of patients with failed hip arthroscopy.
