Abstract
The demographic changes determined a significant increase in the percentage of adults who are very elderly and the proportion of their hospital admissions. In this group, the invasive mechanical ventilation (IMV) use decreased significantly even after adjustment for DNR (do not resuscitate) status. The noninvasive ventilation (NIV) use in the elderly represents a shift in managing acute respiratory failure. The NIV should be used with the correct indications. In choosing the setting, the physicians should take into account the safety of patients.
