Abstract
Endoscopic ultrasound (EUS) is an advanced endoscopic procedure requiring a steep learning curve and great technical and cognitive skills, including an understanding of the proper indications, the performance of appropriate assessments before and after the procedure, and the ability to manage all the procedure-related complications. It has been recently established as an essential modality for the diagnosis and management of pancreato-biliary and gastrointestinal (GI) diseases. An increasing demand for skills to master a growing portfolio of more and more complex interventional EUS procedures and the persisting shortage of formal EUS training programs are two serious impediments to the implementation of an efficient EUS training. EUS competency often requires training beyond the scope of a standard GI fellowship program, dedicating additional education time. In addition to traditional formal EUS training and preceptorships, regular short-term intensive EUS training programs at different levels may help endosonographers improve and maintain EUS knowledge and skills. There are some differences in the way to face the issues of EUS training and education between Europe, America and the Asian regions. Essentially today the right way to EUS training is based on several necessary steps: (1) select the right guy for EUS training. (2) acquisition of the theoretical knowledge (lectures, slides, textbooks, atlases, papers, manuals, DVDs, educational videos, ad hoc websites etc.). (3) simulation-based training using different types of models and simulators. (4) supervised clinical training on real patients. The learning process and progress may vary widely among trainees and this means that a predefined number of procedures during the training experience does not automatically ensure the achievement of adequate EUS skill.
So we must learn how to assess EUS competence mainly using some validated assessment tools or a point score system. The ultimate aim is to guarantee the availability of a sufficient number of well-trained endosonographers or better advanced bilio-pancreatic interventional endoscopists, proficient in both EUS and ERCP.
