Abstract
In this book, we have proposed a framework for understanding the institutions and facilities that are called “hospitals”, noting their variation across the globe. Hospitals concentrate combinations of qualified staff and specific investments in order to take care (especially at diagnosis and treatment level) of the more severe groups of patients. They are a core element of each country’s response to health needs, involving many stakeholders. They deliver a sizeable share of the education and training of health sector staff, and support the majority of the research which underlies medical progress: in all cases, much more than any other setting. Governance is key; even in state systems, the often-unclear business model determines use of resources, necessarily including relevant models of care. Ownership and the payment mechanism are important contingencies. We need more systematic ways to make decisions about how to direct these institutions.
