Abstract
Home-based medical care providers often care for patients receiving in-home functional rehabilitation from a skilled home health care agency. Establishing realistic goals for rehabilitation requires careful assessment of the patient’s medical conditions and the home environment and an understanding of the patient’s and caregiver’s goals of care. Optimal outcomes depend on clear, timely, and effective communication among the entire home-based care team, including the patient and caregiver, skilled therapists, and medical provider.
